Northwest High School

• Authorization to Release Records Form (a link to this form is also available on the Transcript Requests Google Form). This form must be completed in addition to the Transcript Request Google Form.  A signed hard copy of this form must be submitted to the Counseling Office before transcript requests can be completed. 
• Transcripts:
  • Initial Transcript Requests Google Form
  • Mid-Year Transcript Requests:  Mid-year transcripts will be available to be sent from our registrar's office around the middle of February.  Do not use the form for INITIAL transcript requests. 
  • Final Transcripts are sent based on the information shared in your Senior Survey. Final Transcripts are not available until the first week of July. This is an MCPS timeline and not a Northwest timeline, so we ask for your patience and understanding as we wait for Central Office to process and upload all transcripts. 
     
    Again, final transcripts are not available until the first week of July. However, you may begin to receive emails from your attending college/university/organization prior to July. Do not be alarmed. They send out mass emails to all students and are aware that MCPS has a later timeline. You are also welcome to email their admissions team to let them know the expected date that your final transcript will be sent. 
• Scholarship Transcript Request Form
• Selective Service Registration: For high school male students age 17 years and 3 months. Registration is quick, easy, and it's the law.  Registrations processed on the 18th birthday.

Alumni Transcript Requests:

• If you are a former Northwest student that graduated within the last 5 years and need your transcript, please complete the Former Student Transcript Request Form
• If you graduated more than 5 years ago, Northwest no longer has your records and you must contact the Central Records Unit at CentralRecords@mcpsmd.org or visit the Central Records Unit website.
